TICONDEROGA HEALTH CENTER

The Ticonderoga Health Center brings a ‘medical home’ to the Ticonderoga Campus of Elizabethtown Community Hospital. This project involved a renovation and addition of an existing clinic, providing a new entrance to the building. The clinic features 16 exam rooms in a 1-story building and a generous staff work area with exterior views that allows providers and support staff to work side-by-side.

The consolidation of services, now complete with Primary Care, will provide clear one-stop-shopping for all of the community’s medical needs. This project also included a Community Pantry within a healthcare setting where food-insecure individuals can work with their providers to access free and healthy food options.

This project has allowed Hudson Headwaters Health Network to relocate their existing Ticonderoga clinic onto the Elizabethtown Community Hospital’s Ticonderoga Campus, providing direct access to the recently renovated outpatient clinics, lab, and imaging facilities, along with renovated access to the existing skilled nursing facility on campus.
